Dream Theater drummer Mike Portnoy recently suffered an onstage health scare during the band’s Monterrey, Mexico, concert this week. However, he said that he was able to return and complete the performance. The incident happened midway through the set before the encore, as the group continued its 2026 tour run through Latin America.

During the show at Arena Monterrey on April 8, Portnoy said he was hit with sudden nausea and “ended up throwing my guts up before the encore,” adding that it was his first time getting sick onstage in thousands of performances.

Portnoy described the Monterrey date as the “final leg kick-off” of the tour and thanked fans for the strong reception despite the uncomfortable moment. He also said he was feeling better the next day and expected to be able to play the following stop in Mexico City.

“Really nice message written on my hotel room window here in Mexico City! 🙏 Gracias to the thoughtful hotel employee who left the message…(but shhhh, please don’t give away my hotel alias! 🤫😝)

“Thanks to everyone in Monterrey for the great final leg kick-off last night…was a great show! (Not so) fun fact: about halfway through last nights show, I was overwhelmed with a horrible nausea and ended up throwing my guts up before the encore! 🤮 My first time in over thousands of shows actually getting sick on stage during a show!! Luckily feeling better today! ❤️‍🩹 Should be fine to rock Mexico City tomorrow!”

Dream Theater’s schedule has kept the band busy on the road while continuing to build momentum after Portnoy’s return to the group, and the drummer’s quick rebound will be welcome news for ticket-holders. Separate touring concerns have affected other legacy acts, with Rush concerts facing slow ticket sales in a different corner of the live market.

For Dream Theater, Portnoy finishing the set in Monterrey underscores how tightly the band’s current run is paced, leaving little margin for setbacks. If he remains well, the Mexico City performance should proceed as planned, keeping the tour on track through its next dates.
